Work Cards

Work card issuance is regulated by Clark County Codes, City of Las Vegas Municipal Codes, and Nevada Revised Statutes.  The purpose of the work card is to regulate businesses, trades and professions that require a high degree of supervision.

If a work card is required for a position, the employer will supply a work card application to the applicant.  Employers must appear in person at either of the two LVMPD Fingerprint Bureau locations to obtain work card applications to provide to employees.  This application must be completed by the employee and signed by the employer prior to the applicant appearing at the LVMPD Fingerprint Bureau.  Complete instructions are provided on the back side of the work card application.  Common questions and concerns regarding the work card application can be found on the sample application at this link:  Sample Work Card Application.

We accept CASH only. No debit or credit cards can be processed at this time. An ATM is available in the lobby at both of our facilities.

We have two locations to serve you which are open Monday through Friday from 8:00 am until 5:00 pm, closed weekends and holidays.  You may call us during normal business hours at 702-828-3271. After a brief recorded message, an operator can assist you with questions that may not be addressed here.

WORK CARD APPLICANTS

• Applicants with long or curved fingernails may be required to trim their nails prior to being fingerprinted, in order to prevent damage to our equipment. 

• Forms will not be accepted without a signature from the employer. 

• Fill out forms in BLACK INK only. 

• Forms must be completely filled out prior to arrival. 

• Two of the following forms of unexpired ID must be shown (One of the two ID’s MUST include a photo): 

  • - State issued driver’s license or identification
  • - Certified Birth Certificate - Persons 25 years of age and younger must show an original or certified copy of their birth certificate as one form of identification. NO EXCEPTIONS 
  • - Social Security Card 
  • - U.S. Passport 
  • - Military ID 
  • - Naturalization Certificate 
  • - INS Authorization to Work 

• Persons born outside of the US or US territory must show one of the following: 

  • - Naturalization Certificate 
  • - U.S. Passport 
  • - U.S. Birth Certificate 
  • - INS Authorization to Work 

• PHOTOCOPIES OF ANY FORM OF IDENTIFICATION WILL NOT BE ACCEPTED 

• Juveniles 17 years of age and younger must bring a work permit from Clark County Juvenile Justice Services, 601 N. Pecos, 702-455-5200. 

• Child care work cards issued for applicants 17 years of age and younger will expire on their 18th birthday, whereupon a new card and background check will be required. 

• Work cards may not be issued to persons with felony convictions. Each ordinance and statute has varying criteria for work card issuance. 

  • - If you are unsure if you qualify, you may apply and we will review your eligibility. 
  • - If you are denied, we will provide instructions for appeal proceedings if applicable. 

• Work cards may not be issued to persons who falsify the application.
